Facial acne is the number one aesthetic concern for many teenagers but also for many adults. However, back and shoulder acne is much more common than many people think.
Shoulder/back acne can be quite a distressing for people who suffer from it, as it can be widespread on the entire shoulders and/or back area, becoming an embarrassment on the beach and preventing people from wearing open back and/or open shoulder clothing.
Back acne - and also shoulder acne too - can become inflamed, with the inflammation resulting in scarring and/or post-inflammatory hyperpigmentation, especially in people with darker skin.

What causes acne on the back and shoulders
Acne pimples (papules and pustules) on the back and shoulder areas form due to clogged pores. Clogged pores can be caused by a excess oil, dead skin cells, sweat and even friction from tight clothing, usually but not exclusively from man-made fibres (polyester etc).

The main non-friction causes of shoulder/back acne:

Acne reduction on the back and shoulders: what works and what doesn’t
Back and shoulder acne can be controlled to some extent with simple lifestyle changes, such as:
Wearing loose-fitting, breathable, cotton clothing
Washing the back daily with a gentle soap
Gently (never aggressively) exfoliating with a loofah and/or with a gentle alpha hydroxy acid (AHA) formulation
Avoid picking the pimples
Nutritional changes: avoiding fried/hydrogenated fats, sugar, excessive carbohydrates, excessive saturated fats, fruit juices and milk and focusing on consuming plenty of vegetables, non-sweet fruits (e.g. berries) and omega-3 fatty acids
Topical anti-acne formulations (based on benzoyl peroxide, salicylic acid, retinol etc) can also help.
Oral retinoids, oral contraceptives, topical cortisone medications and topical strong retinoids can also be prescribed. However, all those medications can have unwanted adverse reactions and side effects, so they should be reserved for the most severe cases.

Blue LED light acts on porfyrins inside the C. Acnes (Cutibacterium acnes) bacteria, responsible for acne, which results in killing or deactivating them.
Red and infrared LED light act on the mitochondrial enzyme cytochrome c oxidase, boosting mitochondrial function (mitochondria are the powerhouses of the cells) by facilitating the use of oxygen and consequent ATP synthesis (ATP is the energy currency of the cells).

How does blue / infrared / red light therapy work?
Blue, red and infrared light interacts with molecules in the body called chromophores, triggering a cascade of biochemical reactions which result in better cell function. The most important chromophores in the body are hemoglobin, cytochrome c oxidase (Cox), myoglobin, flavins, flavoproteins and porphyrins.
Blue light acts on porfyrins inside the C. Acnes (Cutibacterium acnes) bacteria, responsible for acne, which results in killing deactivating them.
Red and infrared light act on the mitochondrial enzyme cytochrome c oxidase, boosting mitochondrial function (mitochondria are the powerhouses of the cells) by facilitating the use of oxygen and consequent ATP synthesis (ATP is the energy currency of the cells).
